O projeto será implementado pelo CSS, na freguesias de Rio Tinto e Fânzeres, em parceria com Pista Mágica, CM Gondomar, JF Rio Tinto, ACES e JF Fânzeres/SP. O Diagnóstico e Plano de Desenvolvimento Social, Plano Local de Saúde e declaração de elegibilidade demonstram que o território é afetado por problemáticas nos critérios 1, 2, 3, 4 6 e 7. O projeto tem como objetivo promover a saúde e qualidade de vida da comunidade, através de: 1) Criação de uma rede de voluntariado inclusivo, para acompanhamento de proximidade de idosos isolados e que integra como voluntários jovens com deficiência, permitindo que desempenhem uma atividade socialmente útil. 2) Aproximação das populações em maior vulnerabilidade aos cuidados de saúde, através de unidade móvel, promovendo ações de prevenção e rastreio, sobre as principais problemáticas identificadas no PLS, em articulação com o ACES; terapia snoezelen domiciliar para idosos e pessoas com deficiência; programas comunitários de reeducação alimentar.
Beneficiaries
The two types are::
- Direct Beneficiaries are those whose funding and projects to implement are part of the Recovery and Resilience Plan that has been negotiated and approved by the European Union;
- Final Beneficiaries are those whose funding and projects to implement are approved following a selection process through Calls for Applications.
Call for applications
As part of the Call for Applications, submissions are requested to select the projects and final beneficiaries to whom funding will be awarded. Specific selection criteria are defined for each call, which must be reflected in the applications submitted and assessed.
The project is appraised on the basis of its compliance with the selection criteria laid down in the calls for applications, and a final score may be awarded, where applicable.
